ReplyDeletelooking good so far - a couple of things I noticed when I went to complete the survey.
Question 1 needs to be split out as at the moment I can't answer all three quetions you are asking.
When I click other on question four to try and specify there is no free text box to allow this.
You might want to consider (I don't know if your survey tool will allow it) adding a brief satement at the beginning of the survey about what it's for and how long it will take. Just a thought :)

Thanks for your feedback Alex. I haven’t used the ‘surveymonkey’ before and my idea was to take a print out version and distribute to the students. If anyone has the expertise and know how to include the changes that Alex suggested I’d appreciate your help.
